Abstract
PMD is a challenging condition especially when the patient present in an advanced state with high irregular astigmatism, extreme inferior thinning and loss of best corrected visual acuity even by the conservative methods like scleral gas permeable contact lenses. It is vital to know the differential diagnoses for accurate differentiation of PMD from other ectatic conditions. The surgical options can be selected based on whether the PMD is progressing or is stable. These surgical options are different to keratoconus as the risk of ectasia recurrence in peripheral host always remains. This chapter describes these options in detail. Appropriate titration of the management option is vital for successful outcomes in cases with PMD.
