The Role of Arthroscopy in the Management of Fractures Around the Ankle

Abstract

Anatomic restoration and function of the joint is the goal of ankle fracture management. Open reduction and internal fixation (ORIF) is the most commonly used management strategy for unstable ankle fractures. The use of arthroscopy-assisted reduction and internal fixation (ARIF) is increasingly gaining momentum and acceptance among foot and ankle surgeons. Indications for ARIF include fracture of talus, medial and lateral malleolus, low-grade distal tibia fracture (Plafond), syndesmotic injury, paediatric epiphyseal injuries and fractures of the calcaneum. The merits of this procedure include less extensive exposure; preservation of the blood supply to the fracture fragments; improved visualization of the pathology; removal of the bone debris; chondral fragments, which otherwise may go unnoticed using an open reduction method; and most of all, accurate reduction of the articular surface under direct vision. The demerits include specialization in arthroscopy, need for additional equipment set-up, theatre technicians and additional operating time and cost. Most of these fractures are still effectively managed with open procedures.
