Abstract
Gastroparesis is a chronic disorder with a poorly understood pathophysiology. Dietary and lifestyle modification followed by medical treatment are first line treatment but often fail to control symptoms. Severe gastroparesis is an end-organ failure of the stomach which is often best treated with removal of the diseased organ. Roux-en-Y reconstruction with or without gastrectomy offers relief for patients when medical treatments fail. These patients often have many comorbidities which need to be optimized prior to surgery. Although gastrectomy offers significant improvement of symptoms, these patients have a higher rate of complication than other gastrectomy patients.
