Abstract
Disabling obesity is a condition that affects a subgroup of individuals and heavily impairs their physical and psychosocial quality of life. The impact of obesity on disability is particularly severe among the elderly, because in this age range, the physiological reduction of lean mass (sarcopenia) interacts negatively with the effects of obesity. For the treatment of disabling obesity, intensive rehabilitation treatment based on cognitive behavioural treatment for obesity (CBT-OB) should be provided in a specialised unit. This treatment should be delivered by a non-eclectic team composed of physicians, psychologists, dieticians, physiotherapists and nurses, all trained in CBT-OB. It will involve a multidimensional assessment to design a personalised individual rehabilitation plan incorporating nutritional and physical rehabilitation, CBT-OB procedures and medical interventions. In our unit the rehabilitation treatment lasts 21 days and is followed by standard outpatient CBT-OB. The overall aims are to help patients to achieve a healthy weight loss, reduce their cardiovascular risk and obesity-related disabilities, achieve a satisfactory quality of life and develop a lifestyle and mindset conducive to long-term weight maintenance.
