Abstract
Psychotic disorders are the greatest challenge in mental healthcare while, at the same time, they are the emblematic entity of family therapy, its source for inspiration and, historically, its main battle horse.
The relational bases of psychotic disorders are dominated by disconfirmation, a highly destructive phenomenon which, by denying acknowledgement of a person, questions his or her psychological existence and, therefore, attacks their identity. Triangulation is the privileged vehicle of disconfirmation.
The psychotherapeutic approach to psychosis will systematically promote acknowledgement through a reconfirmation process, which will be complemented by different detriangulation measures. Since biological factors must be taken into account and given, in any case, the undeniable medicalization of psychosis, it is important that pharmacological management and the psychiatric dimension be integrated into the psychotherapeutic process.
