Abstract
The Shouldice repair does not call for the use of prosthetic material, and it remains today the gold standard of pure tissue repair for inguinal hernias. This technique applies to both indirect and direct inguinal hernias.
The reconstruction of the posterior inguinal wall is done with four continuous lines using two nonabsorbable sutures. Stainless steel wire sutures are still used (gauge 20–22), but many surgeons use polypropylene sutures equally. This reconstruction makes the posterior wall extremely resistant.
A step-by-step description of the Shouldice repair according to the extant literature, personal experience and actual attendance at the Shouldice Hospital in Toronto is herein provided.
