Case 43: Large Zone III Open Globe from a Finger Injury While Playing Basketball

Abstract

A 57-year-old man presented with a zone III open globe injury of the left eye after being struck with an opponent’s finger while playing basketball. He underwent uncomplicated repair of the open globe. B scan ultrasound on the following day revealed a total retinal detachment and suprachoroidal hemorrhage for which he was promptly referred to a vitreoretinal surgeon. Ten days later he underwent retinal detachment repair which involved a vitrectomy with extensive pre- and sub-retinal membrane peeling, a 360° retinectomy and silicone oil tamponade. His retina remained attached and his vision improved over the course of 2 year follow up. This case illustrates the importance of prompt referral to a vitreoretinal surgeon for management of complicated retinal detachment after trauma.
