Case 6: Dehiscence of Penetrating Keratoplasty from Blunt Trauma

Abstract

A 43-year-old woman presented with a dehisced penetrating keratoplasty graft, expulsed crystalline lens, vitreous loss, and choroidal detachment after blunt trauma to the right eye. The patient underwent immediate repair of the dehisced corneal transplant with interrupted 10-0 nylon sutures under general anesthesia. Her wound was closed successfully and her post-operative care involved treatment for aphakia. She was fitted with a hybrid contact lens and several scleral lenses, but remained bothered by anisometropia and issues with lens maintenance. She is currently considering secondary intraocular lens placement surgery. Other issues regarding traumatic dehiscence of penetrating keratoplasty wounds include infection, irregular corneal astigmatism, and graft failure.
