Reconstruction of the New Umbilicus: Indications and Choices

Abstract

Bariatric surgery has brought a new type of patient into the surgeon’s office, demanding new surgeries. Anchor-line abdominoplasty has now become a routine procedure. The treatment of the umbilicus is a new challenge in those patients. Neo-omphaloplasty is now a common surgical procedure. The author shows a new method to treat the umbilical scar, especially in anchor-line abdominoplasty.

Method: Two flaps are made (1.5 × 2.0 cm each). These flaps have curved bases. They are sutured into the aponeurosis using two parallel points, reconstructing the foundation of the new umbilicus. The periumbilical fat is approached in order to elevate the borders and create the new umbilicus. By doing this, it is possible to reconstruct all of the anatomical structures of the umbilicus (corrugated part, ring, and impeller).

Results: The new umbilicus has a natural aspect, without a stigmatizing scar and with good depth. Since there is no circular scar, the chance of stenosis is lessened.

Conclusion: The reconstruction of the new umbilicus using this technique is easily reproducible with excellent aesthetic results. Good depth and the lack of stenosis is also achieved.
