Abstract

The tragedy of Lion Air Flight 610 and Ethiopian airlines flight 302 offer important insights into the dangers of advancing automation without a complete understanding of the context in which the system will be used. This paper recounts the story of Lion Air flight 610, and provides a brief analysis of the organizational, market, and cultural factors that resulted in the crash. The analysis highlights how a series of failures in each of the factors can compound to result in catastrophic consequences. The lessons learned are applied to the nuclear industry to describe how similar failures could manifest in the nuclear industry if designers, regulators, and operators don’t carefully consider similar factors.
