Multiple Depigmented Macules on Trunk

Abstract

A 55 year old male presented with multiple hypopigmented macules over back and abdomen since 1 year. Dermoscopy showed amoeboid pattern with pseudopod like projection. Based on this information diagnosis of idiopathic guttate hypermelanosis was made. Idiopathic guttate hypomelanosis is a common, acquired benign condition characterized by few or multiple small hypo to depigmented macules of size varying from 2 mm to 2 cm usually seen over shin, forearms but can be seen on other body parts, noticed in middle aged people. Sunlight and repeated trauma play a role in etiopathogenesis. Diagnosis is usually clinical. Dermoscopy reveals characteristic patterns including amoeboid, feathery, petaloid and nebuloid with perimetric pigmentary extensions (pseudopods). Management is usually based on patients concerns, simple reassurance would be enough, if cosmetically worried management options include topical tacrolimus, topical retinoids, spot peel with 88% phenol, dermabrasion, fractional CO2 laser, punch excision.
