Abstract
When attempts to arrest progression of renal insufficiency seem to fail and terminal uremia approaches, renal replacement treatment (RRT) must be planned. All too often such planning has to take place under rushed conditions, the patient being too uremic when first seen by the nephrologist to permit investigation of all aspects before action is taken. Urgent situations should be avoided, because the risk is high that the wrong decision will be made.
