  1. Costs (1998 values) were those related to the intervention (including screening, materials and nurse assessments/supervision), outpatient visits (including emergency-department, medical and mental health visits) and psychotropic drugs. Indirect costs associated with travel and waiting times were also calculated.

  2. Costs included those associated with drug acquisition, clinic and emergency-room visits, and laboratory and diagnostic tests.

  3. 1997 National Asthma Education and Prevention Program updated Expert Panel Report 2 guidelines.

  4. One of the researchers was affiliated with Merck & Co., which provided funding for the study.
