Causal analysis for sodium hydroxide (NaOH) spray from peristaltic pump tube failure

Abstract

Bulking is the process of transferring the contents of several small containers of liquid waste into a single larger container. At Pacific Northwest National Laboratory, bulking is routinely performed to minimize the impact on the environment and to lessen the disposal costs for waste. Using a peristaltic pump to transfer liquid waste from one container to another is efficient and keeps contamination at a minimum. An unanticipated blockage in the pump tubing quick-disconnects caused a failure that resulted in hazardous waste to be sprayed outside of the pump and onto two pump operators. This paper discusses the methodology used for determining the cause of the event.
