Abstract
The basilar web is considered an atypical intimal fibromuscular dysplasia that is even rarer than the carotid web. We present two cases of elderly patients with posterior circulation ischemic stroke. One of them had a paramedian pontine cerebral infarction and the other had a posterior cerebral artery infarction. Both patients underwent magnetic resonance angiography and tomography angiography of the brain, which showed a linear filling defect in the upper part of the basilar artery compatible with basilar web. The angiographic findings of the basilar web are similar to those of the carotid web, and allow it to be differentiated from other alterations such as atherosclerotic plaques or arterial dissection. However, both the epidemiological characteristics and the thromboembolic potential of basilar web patients are uncertain.
