Abstract
The combination of porto-mesenteric venous thrombosis (PVT) and peritoneal abscess is an uncommon complication of uncontrolled intra-abdominal inflammatory condition. The complications are associated with high morbidity and mortality when not recognized and treated early. We present our experience with a case of porto-mesenteric venous thrombosis and peritoneal abscess complicating sigmoid diverticulitis which was diagnosed with contrast-enhanced CT abdomen and managed with fluid therapy, antibiotics, and anticoagulant. This case became unique due to the surprising clinical and laboratory pictures of obstructive jaundice. Our case not only gives a different clinical and laboratory presentation of a known complication of a common disease but also raises an awareness on the wider role of ultrasound in its initial evaluation. Ultrasound evaluation of the portal and mesenteric veins should be incorporated into initial assessment of suspected obstructive jaundice.
