Zusammenfassung
Vor einigen Monaten wurde die „International evidence-based guideline for assessment and management of polycystic ovary syndrome 2018“ auf der ESHRE-Website aktualisiert: Im Unterschied zu der früheren Version wird, neben den endokrinologischen und metabolischen Aspekten der Erkrankung, zusätzlich auf die psychologischen Aspekte eingegangen. Die Diagnostik stellt in den verschiedenen Lebensphasen der Frau, Pubertät – reproduktive Phase – Menopause, eine besondere Herausforderung dar. Ein PCOS manifestiert sich häufig schon in der Adoleszenz, wird aber wegen der Überlappung zu normalen Befunden leicht übersehen. Zur Diagnosestellung gelten die 3 Rotterdam-Kriterien. Das Ultraschallkriterium wurde verschärft (antrale Follikel: 20 versus früher 12). Die polyfollikuläre Ovarmorphologie (PCOM) sollte gemäss „guideline“ neu bis acht Jahre nach Menarche nicht diagnostisch verwertet werden. Ausserdem kann neu bei Patientinnen mit irregulärem Zyklus und Hyperandrogenismus auf die Ultraschalldiagnostik verzichtet werden. Das Therapiekonzept sollte nicht nur symptomorientiert sein, sondern ein umfassendes interdisziplinäres Management mit folgenden weiteren Zielen verfolgen: Verringerung der kardiovaskulären Risiken, Lifestyle-Interventionen, Prophylaxe der Endometriumhyperplasie, Verbesserung von psychischen Beschwerden, Ovulationsinduktion bei Kinderwunsch. Darüber hinaus werden neue First-line-Medikamente präsentiert. Die Beratung der Patientin sollte ein essenzieller Schritt bei der Behandlung sein, um schwere gesundheitliche Folgen möglichst zu minimieren. Die Richtlinien wurden am ESHRE-Kongress 2018 in Barcelona präsentiert und sind auf der ESHRE-Website jederzeit einzusehen.
Résumé
Il y a quelques mois, l’ESHRE a publié une mise à jour des directives internationales «International evidence-based guideline for assessment and management of polycystic ovary syndrome 2018» sur son site Internet. Autrement que dans la version précédente, les directives portent désormais aussi sur les aspects psychologiques de la maladie à côté de ses aspects endocrinologiques et métaboliques. L’établissement du diagnostic dans les différentes phases de la vie féminine – puberté, phase reproductive, ménopause – reste un défi particulier. Le SOPK se manifeste souvent déjà dans l’adolescence, mais peut facilement passer inaperçu à cause de points communs avec des constats normaux aux examens. Le diagnostic s’appuie sur les 3 critères de Rotterdam. Le critère échographique est devenu plus exigeant (désormais 20 follicules antraux au lieu de 12). D’après les nouvelles directives, la présence d’une morphologie ovarienne polyfolliculaire (PCOM) ne doit désormais être prise en compte pour le diagnostic que si la ménarche remonte au moins à huit ans. De plus, on peut désormais renoncer aux examens échographiques lors d’irrégularités menstruelles et d’hyperandrogénisme. Le concept thérapeutique doit non seulement être orienté symptômes, mais aussi viser les objectifs suivants dans le cadre d’une prise en charge complète interdisciplinaire: réduction des risques cardio-vasculaire, adaptation de l’hygiène de vie, prévention de l’hyperplasie de l’endomètre, réduction des perturbations psychologiques, induction de l’ovulation si désir d’enfant. À part cela, les directives présentent de nouveaux médicaments de première intention. Les conseils prodigués à la patiente doivent être un élément essentiel du traitement pour réduire autant que possible les conséquences sérieuses pour la santé.
Les directives ont été présentées dans le cadre du congrès de 2018 de l’ESHRE à Barcelone et peuvent être consultées à tout moment sur le site de l’ESHRE.
