Zusammenfassung
Blutungsstörungen in der Peri- und Postmenopause gehören zu den häufigsten Beschwerdebildern in der gynäkologischen Praxis. Sie machen ca. 70 % aller Konsultationen in dieser Altersgruppe aus. Meist sind hormonelle Dysbalancen infolge der nachlassenden ovariellen Aktivität Ursache der abnormen uterinen Blutungen (AUB) in der Perimenopause. Im Rahmen der Diagnostik sollten neben der Anamnese und klinischen Untersuchung auch eine Vaginalsonographie und ein PAP-Abstrich erfolgen, um eine prämaligne/maligne Ursache ausschließen zu können. Die Therapie richtet sich jeweils nach dem Auftreten (akut/chronisch; hämodynamisch relevant oder nicht), der Ursache der Blutung und der Frage, ob durch die Blutung eine Einschränkung der Lebensqualität besteht. Im Falle einer akuten Blutung stehen die Stabilisierung der Patientin mit Volumenersatz/Bluttransfusion und eine hormonelle/operative Folgeintervention im Vordergrund. Bei chronischen Blutungen mit Einschränkung der Lebensqualität können neben medikamentösen Therapien (hormonell, Antifibrinolytika, nicht-steroidale Antiphlogistika [NSAR]) bei Beschwerdepersistenz oder auch auf Wunsch der Patientin operative Maßnahmen durchgeführt werden.
Abstract
Bleeding disorders in the perimenopause and postmenopause are the most frequent symptoms in the gynecological practice. They constitute approximately 70 % of all consultations in this age group. In most cases the cause of the abnormal uterine bleeding (AUB) is hormonal imbalance as a result of the decline in ovarian activity in the perimenopause. During the diagnostic procedure a vaginal sonographic examination and a PAP smear should be carried out in addition to the patient medical history and clinical examination in order to be able to exclude a premalignant or malignant cause. The treatment is alligned to the occurrence (acute or chronic, hemodynamically relevant or not), the cause of the bleeding and the question whether the bleeding results in an impairment of the quality of life. In the case of acute bleeding the priorities are stabilization of the patient with volume replacement/blood transfusion and a hormonal/surgical intervention. In cases of chronic bleeding with impairment of the quality of life, in addition to medicinal treatment (hormonal, antifibrinolytics, nonsteroidal anti-inflammatory agents) for persisting complaints, operative measures can also be carried out if desired by the patient.
Résumé
Les saignements utérins anormaux dans la périménopause et la postménopause font partie des symptômes les plus fréquents en gynécologie. Ils sont la cause d’environ 70 % de toutes les consultations dans cette tranche d’âge. Ce sont le plus souvent des déséquilibres hormonaux dus au déclin de l’activité ovarienne qui causent les saignements utérins anormaux (SUA) dans la périménopause. À côté de l’anamnèse, le processus de diagnostic doit aussi inclure une échographie vaginale et un frottis cervico-utérin pour pouvoir exclure une cause précancéreuse ou cancéreuse. Le traitement dépend du type de saignement (aigu/chronique, hémodynamiquement significatif ou non), de sa cause et de la question si le saignement affecte la qualité de vie. Dans le cas d’un saignement aigu, le premier objectif est de stabiliser la patiente par un traitement de remplissage vasculaire/transfusion sanguine, suivi d’une intervention hormonale/chirurgicale. Lors de saignements chroniques détériorant la qualité de vie, on peut prescrire des traitements médicamenteux (hormonaux, antifibrinolytiques, AINS) et, si les symptômes persistent ou si la patiente le souhaite, recourir aussi à des mesures chirurgicales.
