Zusammenfassung
Als Standarduntersuchung zur Prostatakarzinomdiagnose gilt die transrektale Biopsie. Das Infektionsrisiko steigt jedoch mit zunehmender Antibiotikaresistenz der Darmflora. Wir präsentieren eine standardisierte, transperineale Biopsietechnik, welche seit Januar 2015 in unserer Praxis routinemäßig wie folgt angewendet wird: Prophylaxe mit 500 mg eines oralen Fluorquinolons, Steinschnittlage und Desinfektion des Perineums; rektales Einführen einer linearen Ultraschallsonde zur Bildgebung der Prostata sowie Sichtkontrolle der gesamten Prozedur; Lokalanästhesie durch Injektion von je 10 ml 1 %igem Lidocain beidseits und ventrolateral des Anus. Haut, Subkutis, Beckenboden und die Prostatakapsel werden betäubt. Eine Metallkanüle wird als Zugangshülse in die betäubte Haut vorgelegt. Durch diese erfolgt die standardisierte Gewebeentnahme der anterioren, mittleren und dorsalen Prostata. Bei Patienten mit vorgängiger multiparametrischer Magnetresonanzuntersuchung (mpMRT) werden zusätzlich kognitiv gezielt Proben entnommen. Vor Entlassung erfolgt eine Schmerzbewertung der Patienten auf einer 10-stufigen Skala. Bisher wurden 212 Patienten in einem mittleren Alter von 66,3 (49–84) Jahren biopsiert; davon 176 (83 %) mit vorliegendem mpMRT; der mediane PSA-Wert betrug 5,9 (0,3–600) ng/ml. Im Median wurden 12 (4–18) Proben entnommen. Es gab keine Infektionen oder stationäre Aufnahmen. Makrohämaturieepisoden waren selbstlimitierend. Zwei Patienten (0,9 %) erlitten einen Harnverhalt. Der mediane Schmerzscore lag bei 2,0 (0–8), die Krebsdetektionsrate bei 61,8 %. Die Kosten und der zeitliche Aufwand waren mit denen der transrektalen Technik vergleichbar. Die transperineale Entnahme könnte sich auch im niedergelassenen Bereich als kostenneutrale, effiziente und sichere Alternative zur transrektalen Entnahme durchsetzen.
Résumé
La biopsie transrectale est considérée comme un examen standard pour le diagnostic du cancer de la prostate. Le risque d’infection augmente cependant avec une résistance croissante de la flore intestinale aux antibiotiques. Nous présentons une technique de biopsie transpérinéale standardisée qui est appliquée systématiquement comme suit depuis janvier 2015 chez nos patients: prévention par 500 mg d’une fluoroquinolone orale, installation du patient en position de lithotomie et désinfection du périnée; introduction rectale d’une sonde échographique linéaire pour l’imagerie de la prostate et pour le contrôle visuel de toute la procédure; anesthésie locale par trois injections de 10 ml de solution de lidocaïne à 1 % (à droite, à gauche et en zone ventro-latérale de l’anus). La peau, l’hypoderme, le plancher pelvien et la capsule prostatique sont anesthésiés. Une canule métallique est insérée en tant que canal d’accès dans la peau anesthésiée. Le prélèvement standardisé de tissu de la prostate en zone antérieure, centrale et dorsale est effectué à travers ce canal. Chez les patients soumis auparavant à un examen par IRM multiparamétrique (IRMmp), des échantillons supplémentaires sont prélevés de façon ciblée. Une évaluation des douleurs sur une échelle de 10 points est faite avant la sortie du patient. Ces biopsies ont été effectuées jusqu’à présent chez 212 patients âgés en moyenne de 66,3 (49 à 84) ans, dont 176 (83 %) avec IRMmp préalable; le taux moyen de PSA était de 5,9 (0,3 à 600) ng/ml. Un nombre médian de 12 échantillons (4 à 18) a été prélevé. Il n’y a eu aucune infection et aucune hospitalisation. Les épisodes de macrohématurie étaient autolimités. Deux patients (0,9 %) ont subi une rétention urinaire. Le score médian de douleur était de 2,0 (0 à 8), le taux de détection de cancers était de 61,8 %. Les coûts et le temps consacré étaient similaires à ceux de la technique transrectale. Le prélèvement transpérinéal pourrait s’établir aussi dans le domaine extrahospitalier en tant qu’alternative efficace, sûre et neutre sur le plan des coûts par rapport au prélèvement transrectal.
