Zusammenfassung
Die sakrale Neuromodulation (SNM) wurde in den letzten Jahren in Urologie sowie Proktologie etabliert. Die Indikation ist in verschiedenen Leitlinien als wertvolle Therapieoption festgelegt, nachdem konservative und medikamentöse Therapieversuche fehlgeschlagen sind. In einem stufenweisen Vorgehen wird die Effektivität der SNM an den Nervenwurzeln – zumeist S3 – in einer individuell auf den Patienten abgestimmten Testphase (Tage bis Wochen; Verbesserung ≥50 %) evaluiert. Bei erfolgreichem Testergebnis wird ein permanenter Schrittmacher implantiert. Die positive Wirkung der SNM über viele Jahre ist bisher in zahlreichen Publikationen belegt. Daher sind die hohen Kosten gerechtfertigt. Allerdings ist ein Austausch der Schrittmacherbatterie nach 5–8 Jahren erforderlich. Die Lebensqualität vieler Patienten wird durch SNM deutlich gebessert.
Abstract
In recent years sacral neuromodulation (SNM) has been established both in urology and proctology. In various guidelines the indication is defined as a valuable therapeutic option, when attempts at conservative and drug therapy have failed. In a stepwise approach, the effectiveness of SNM at the nerve roots—usually S3—is evaluated in a test phase (days to weeks, improvement ≥50%) that is tailored to the individual patient. If testing is successful a permanent pacemaker is implanted. To date, numerous publications have confirmed the positive effect of SNM. Therefore, its high costs are justified. However, replacement of the pacemaker’s battery is necessary after 5–8 years. SNM has clearly improved quality of life in many patients.
