Luxatiefractuur van de enkel met posterieure verplaatsing van de fibula: de Bosworthfractuur

Samenvatting

Een luxatiefractuur van de enkel met verplaatsing en opsluiting van de proximaal van de fractuur gelegen fibula naar posterieur, de bosworth-fractuur, wordt slechts incidenteel gezien. In dit artikel wordt een casus gepresenteerd, gevolgd door een beschouwing van de diagnose, behandeling en complicaties van deze zeldzame luxatiefractuur van de enkel. Vroege herkenning en directe repositie van de geluxeerde enkel zijn essentieel. In het geval van een Bosworth-fractuur is gesloten repositie veelal onmogelijk en daarom dienen open repositie en interne fixatie (ORIF) de behandelstandaard te zijn.

Abstract

A fracture dislocation of the ankle with posterior displacement and entrapment of the proximal fibula, the Bosworth fracture, is only very rarely seen. In this article a case presentation is followed by a consideration of the diagnosis, treatment and complications of this uncommon fracture of the ankle. Early recognition and immediate reduction of the dislocated ankle are essential. In case of a Bosworth fracture closed reduction is usually impossible and therefore open reduction and internal fixation (ORIF) are considered the treatment of choice.
