Samenvatting
Letsel van de nervus radialis treedt in wisselende mate van ernst op bij circa 10% van de patiënten met een gesloten midschacht-humerusfractuur. Vroege chirurgische interventie levert een goede kans op herstel, evenals primair afwachten met eventuele late chirurgische interventie. De behandeling van keuze is hiermee in grote mate afhankelijk van de voorkeur van patiënt en behandelaar. Echografische beoordeling van de nervus in de acute fase kan een aanvulling zijn in het diagnostisch proces. Een intacte nervus wordt vastgelegd en onnodige chirurgische exploratie wordt voorkomen; patiënt en behandelaar verkrijgen een groter gevoel van zekerheid tijdens de expectatieve periode.
Abstract
In 10% of all patients with a closed midshaft humeral fracture, radial nerve injury emerges in varying degrees of severity. Early surgical intervention and expectative treatment with possible late surgical intervention show an equally good recovery rate. Therefore, the decision primarily depends on the preference of patient and therapist. The diagnostic process can be supplemented by ultrasound evaluation of the nerve in the acute phase. Subsequently, an intact nerve can be detected, avoidable surgical interventions can be prevented and patient and therapist can be given a more secure feeling during the expectative period.
