Een training in gedeelde besluitvorming

Samenvatting

Doel We onderzochten het effect van een training in gedeelde besluitvorming voor huisartsen bij patiënten met lagerugpijn, waarbij de arts ook de verwachtingen van de patiënt over de gekozen aanpak bekrachtigde.

Methode Voor een clustergerandomiseerde trial beoordeelden we video-opnamen van 86 consulten door 23 artsen die de traning volgden en 89 consulten door 19 ongetrainde artsen. Onze primaire uitkomsten waren de score op het instrument Observing Patient Involvement in Decision Making (OPTION), plus een globale inschatting van de mate van positieve bekrachtiging. Secundaire uitkomstmaten waren autonomie van de patiënt in de besluitvorming, bespreken van het gunstige spontane beloop en consultduur.

Resultaten De consulten in de interventiearm scoorden significant hoger op de meeste OPTION-elementen en op autonomie, maar duurden drie minuten langer. Het niveau van de gedeelde besluitvorming bleef onbevredigend.

Conclusie De training leidt tot meer gedeelde besluitvorming en tot meer autonomie van de patiënt, maar dat is vooral het gevolg van betere informatieverstrekking; er is niet meer aandacht voor het perspectief van de patiënt. Het meetinstrument voor positieve bekrachtiging van verwachtingen bleek onbetrouwbaar.

Praktische aanbeveling In de toepassing van gedeelde besluitvorming, moeten professionals meer aandacht geven aan de verwachtingen van patiënten over hun behandeling.
