Samenvatting
Het eindstadium hartfalen is een complex klinisch syndroom met een onzekere levensverwachting, een grillig ziektebeloop met exacerbaties en een slechte prognose. Het gaat vaak gepaard met comorbiditeit. De symptoomlast bij hartfalen is erg groot. De levensverwachting is matig voorspelbaar en advance care planning komt weinig aan bod. Ziektegerichte behandeling geeft vermindering van klachten en gaat bij hartfalen hand in hand met symptoomgerichte palliatieve behandeling. Zorg gericht op palliatie heeft nog weinig aandacht door gebrek aan scholing en gebrek aan afstemming in de keten. Dit leidt vaak tot ad-hoc crisiszorg.
Hartfalenmedicatie in het eindstadium wordt zo lang mogelijk gecontinueerd en nauwkeurig getitreerd op geleide van bloeddruk, mate van over- of ondervulling en nierfunctie. De shockfunctie van de ICD/CRT/pacemaker in de laatste levensfase wordt tijdig gedeactiveerd na zorgvuldig overleg met patiënt en familie en instemming van patiënt. Onder strikte criteria komt een kleine groep patiënten in aanmerking voor intraveneuze thuisbehandeling voor hartfalen met diuretica en soms inotropie. Goede communicatie en organisatie voor naadloze zorg tussen de ketens is essentieel en ontwikkelt zich landelijk (https://www.nhg.org/actueel/nieuws/landelijke-aanpak-voor-organisatie-van-zorg-bij-hartfalen).
