Regional Variation in Technetium Pyrophosphate Uptake in Transthyretin Cardiac Amyloidosis and Impact on Mortality

Context: Technetium 99 m-pyrophosphate (Tc-PYP) scintigraphy is increasingly being used as the diagnostic standard in cardiac ATTR amyloidosis. Regional variation in myocardial Tc-PYP has been reported. Longitudinal strain (LS) echocardiography has shown an apical sparing distribution of strain abnormality in cardiac amyloidosis.

Methods and Results:To investigate the association between regional uptake of Tc-PYP in transthyretin cardiac amyloidosis (ATTR) and mortality, the authors performed a retrospective analysis of 54 patients with ATTR (age 78 ± 9 years, 76% male and 31% with hereditary ATTR).
