Zusammenfassung

Nach der Aortenklappenstenose stellen AV-Klappeninsuffizienzen (AV: atrioventrikular) die häufigsten Herzklappenfehler dar. Ätiologisch unterteilt in primäre und sekundäre Insuffizienzen ist die Behandlungsstrategie grundlegend unterschiedlich. Dies betrifft die Bedeutung und Dauer einer medikamentösen Therapie, den Zeitpunkt einer Intervention, die Frage nach Ersatz oder Rekonstruktion und die Wahl eines chirurgischen oder katheterinterventionellen Verfahrens. Im vergangenen Jahrzehnt hat sich die kathetergestützte Therapie besonders dynamisch entwickelt. So werden inzwischen mehr isolierte Mitralklappeninsuffizienzen mit einem katheterinterventionellen Verfahren behandelt als mittels rekonstruktiver Chirurgie. Auch sind mittlerweile erste kathetergestützte Behandlungen der Trikuspidalklappeninsuffizienz durchgeführt worden. In diesem Positionspapier werden die aktuell zur Verfügung stehenden katheterbasierten Verfahren zur Behandlung der Insuffizienz von Mitral- und Trikuspidalklappe zusammengefasst und ihr gegenwärtiger Stellenwert für die klinische Versorgung dargestellt. In Anbetracht der Prävalenz der Mitralinsuffizienz und der hohen Anzahl von in Deutschland durchgeführten Prozeduren an dieser Klappe hat sich die Deutsche Gesellschaft für Kardiologie entschieden, Qualitätskriterien für eine Zertifizierung von Zentren für die katheterinterventionelle Therapie der Mitralklappe zu definieren.

Abstract

Mitral and tricuspid valve regurgitation remain among the most prevalent valvular defects. The treatment strategy critically depends on the etiology of disease and differs with respect to the role of medical therapy, the timing of intervention, the strategy of whether to replace or repair, and the use of a transcatheter approach. Here, we summarize currently approved transcatheter therapies for the treatment of mitral and tricuspid regurgitation. Given the prevalence of disease and the high and increasing number of transcatheter procedures in particular for mitral valve regurgitation, the German Cardiac Society herewith publishes criteria for centers for transcatheter mitral valve therapy.
