Résumé
Objectifs
Évaluer les résultats du traitement chirurgical du cancer de l’estomac.
Méthodologie
Étude rétrospective de janvier 1999 à décembre 2008 portant sur tous les malades opérés pour cancer gastrique dans le service de chirurgie générale du CHU Gabriel Touré. Les variables étudiées ont été: l’âge, le sexe, les stades d’évolution et les suites opératoires.
Résultats
Nous avons enregistré 342 cas de cancer gastrique soit 53,7% des cancers digestifs. Deux tiers des malades étaient de sexe masculin. L’âge moyen était de 51,8 ans (12,9). Chez 316 malades (94 %) le cancer était diagnostiqué au stade III et IV. Le siège antral a été le plus fréquent 298 cas (87,1 %). L’échographie abdominale et la radiographie thoracique de face ont été réalisées chez tous les patients. Elles ont révélé 38 métastases hépatiques, 58 cas d’ascite et 6 métastases pulmonaires. La chirurgie palliative a été effectuée chez 167 patients (153 fois la gastro-entéroanastomose, 12 gastrostomies d’alimentation et la jéjunostomie 2 fois). La chirurgie à visée curative a été réalisée chez 84 malades (24,6 %). Chez 13 patients seule une biopsie a été possible à la laparotomie. La morbidité postopératoire a été de 9,8% soit 21 cas d’abcès de paroi, 3 éviscérations et 2 fistules digestives externes. La mortalité postopératoire immédiate a été de 3,8 % (10 cas). Parmi les 78 malades non opérés 61 (78,2 %) sont décédés au cours du 1er mois du diagnostic. La durée moyenne d’hospitalisation a été de 14,6 jours (9,5 jours). Le taux de survie à 5 ans a été de 20,9 % après chirurgie curative.
Conclusion
Le cancer gastrique est le premier cancer en chirurgie digestive au Mali, son pronostic reste mauvais à cause du diagnostic tardif. Le cancer de l’estomac est le 2e cancer le plus fréquent au Mali. Sa prise en charge est essentiellement chirurgicale.

Objectives
Evaluate the results of the surgical treatment of gastric cancer.
Methodology
We carried out a retrospective study (January 1999 to December 2008) related to all the patients operated for gastric cancer in the department of general surgery at the teaching hospital Gabriel Touré in Bamako.
Results
We recorded 342 cases of gastric cancer witch represented 53.7% of digestive cancers. Two thirds of the patients were male. The mean age was 51.8 (12.9 years). For 316 patients (94%) cancer was diagnosed at the stage TNM III and IV. Antral tumours was more frequent 298 cases (87.1%). Abdominal ultrasonography and the thoracic plain radiography were carried out among all patients and revealed: 38 hepatic metastases, 58 cases of ascite and 6 pulmonary metastases. Gastrectomy with lymphadenectomy was carried out among 84 patients (24.6%). The morbidity after surgery was 9,8%, the complications was 21 case of surgical site abscess, 3 eviscerations at and 2 external digestive fistula. The mortality after surgery was 3.8% (10 cases). Among the 78 non operated patients, 61 (78.2%) died during the 1st month of the diagnosis. The mean duration of hospitalization was 14.6 days (9.5 days). The rate of survival at 5 years was of 20.9% after curative surgery.
Conclusion
Gastric cancer is the first cancer in digestive surgery in Mali, its forecast remains bad because of the late diagnosis. Gastric cancer is the 2nd most frequent cancer in Mali.
