Zusammenfassung
Die außerklinische invasive und nichtinvasive Beatmung ist eine Therapieform bei respiratorischer Insuffizienz unterschiedlichster Genese. Sie kann im eigenen häuslichen Umfeld, in speziellen Beatmungswohngemeinschaften oder Beatmungsheimen durchgeführt werden. In den letzten Jahren stieg die Zahl an außerklinisch beatmeten Patienten, weshalb dieses Thema an Bedeutung gewinnt. Aufgrund physiologischer Veränderungen der Ventilation während des Schlafs manifestiert sich eine ventilatorische Insuffizienz häufig zuerst im Schlaf. Durch die reduzierte Muskelaktivität, insbesondere während des REM-Schlafs, kommt es besonders bei Patienten mit Schwäche der Atemmuskulatur zu einer Zunahme der nächtlichen Hypoventilation. Unter einer suffizienten Beatmung konnte eine Verbesserung der Schlafarchitektur, eine Abnahme der Tagesmüdigkeit und eine Zunahme der subjektiven Schlafqualität nachgewiesen werden. Neben der Schlafqualität ist auch die Erhaltung bzw. Verbesserung der gesundheitsspezifischen Lebensqualität ein maßgebliches Therapieziel bei außerklinisch beatmeten Patienten. Es wurde gezeigt, dass die Lebensqualität bei Patienten mit unterschiedlichen Erkrankungen und auch mit chronisch obstruktiver Lungenerkrankung (COPD) unter einer nichtinvasiven Beatmung gesteigert werden kann. Hervorzuheben ist aber auch, dass eine invasive Beatmung gegenüber einer Maskenbeatmung keine schlechtere Lebensqualität vorweisen muss. Patienten mit neuromuskulären oder thorakal-restriktiven Erkrankungen scheinen dabei eine höhere gesundheitsbezogene Lebensqualität als Patienten mit primär pulmonalen Erkrankungen aufzuweisen.
Abstract
Invasive or noninvasive home mechanical ventilation is a therapy for respiratory insufficiency of various origins. It can be used in the domestic environment, respiratory special care units, or nursing homes. Due to the fact that the number of patients receiving home mechanical ventilation has increased in recent years, this topic is becoming more and more important. The physiological changes in ventilation during sleep mean that respiratory insufficiency often only manifests during sleep. Due to the reduced muscle activity, particularly during REM sleep, patients with reduced respiratory muscle strength show increased nocturnal hypoventilation. With sufficient ventilation, an improvement in sleep architecture, a reduction in daytime sleepiness, and an improvement in subjective sleep quality could be demonstrated. Not only is sleep quality important, but conservation/improvement of the health-related quality of life of home mechanical ventilation patients is also a major therapeutic target. It could be shown that the quality of life of patients with different diseases, including chronic obstructive pulmonary disease (COPD), can be improved by noninvasive ventilation. However, it must also be emphasized that the health-related quality of life of patients receiving invasive ventilation need not be lower than that of patients with mask ventilation. Patients with neuromuscular or thoracic-restrictive diseases seem to have a higher health-related quality of life than patients with primary pulmonary diseases.
