Abstract
Failure of a telescopic shiploader during installation was investigated by using visual inspection, optical and electron microscopy, fracture toughness and tensile tests, and finite element calculation. Failure of this structure occurred at the time of loading the balance weight during installation. After putting 77 of the 80 tons of the balance weight, two nonprismatic beams on the upper part of the structure failed suddenly. The results of the failure analysis show that brittle fracture of one of the main horizontal beams acted as a main initiating factor for failure of the structure. These results indicate that brittle behavior of this beam occurred because of microcracks developed during one of the several welding processes made on this beam. The welding produced abnormal microstructure and cracks in the damaged area of the beam. The combination led to fracture and failure of the beam.
