Zusammenfassung
Für die schrittweise nichtinvasive Diagnostik von Nierentumoren stehen 5 verschiedene Techniken zur Verfügung. Während die B-Bild- und die Farbduplexsonographie als primäres Screening eingesetzt werden, findet die Computertomographie als Verfahren der Wahl Anwendung für die weitere Dignitätsbeurteilung und die Planung des therapeutischen Vorgehens. Die Magnetresonanztomographie hat ihren Schwerpunkt in der Diagnostik zystischer Nierentumoren und steht zur Verfügung bei kontrastmittelabhängigen Kontraindikationen für die CT. Für die MRT liegt allerdings seit neuestem bei deutlicher Einschränkung der Nierenfunktion (GFR <30 ml/min) aufgrund der Gefahr einer nephrogenen systemischen Fibrose eine Kontraindikation vor. Die Anwendung der Positronenemissionstomographie beschränkt sich zurzeit noch auf Patienten in der Verlaufsbeobachtung oder Patienten mit verdächtigten Metastasen. Zu den neueren Entwicklungen zählt nicht nur die verbesserte Darstellbarkeit von renalen Raumforderungen durch Ultraschallkontrastmittel, sondern auch die Eingruppierung anhand des Kontrastmittelverhaltens im CT. Zudem wird in neuerer Zeit bei bestehenden Zweifeln zur Dignität die Ultraschall- oder CT- gesteuerte Biopsie der Tumoren empfohlen.
Abstract
Noninvasive imaging of renal masses has improved in recent years. Five different techniques are now available for a stepwise diagnostic procedure. While renal ultrasound and color Doppler sonography are used for tumor screening, computed tomographic angiography remains the imaging technique of choice for grading and classifying cystic and solid renal masses. In patients with allergies to iodinated contrast media or with renal insufficiency, magnetic resonance tomography (MRT) can play a major role, especially for cystic renal masses. However, nephrogenic systemic fibrosis has been recently described in more than 200 cases after MRT-associated application of gadolinium-based contrast media, thus currently limiting the use of MRT. Positron emission tomography should be limited to patients under surveillance or with suspected metastatic lesions. Recent developments in imaging techniques with new classifications to improve diagnostic accuracy are discussed in this article.
