Zusammenfassung
Die Diagnose funktioneller Abdominalbeschwerden im oberen (funktionelle Dyspepsie, „Reizmagen“) und unteren Gastrointestinaltrakt (Reizdarmsyndrom) ist erst gesichert nach differenzialdiagnostischem Ausschluss von organischen Erkrankungen. Insbesondere bei Vorliegen von Alarmsymptomen verbietet sich eine initiale probatorische Therapie bei dyspeptischen Symptomen und/oder Stuhlgangsveränderungen.Die Basisdiagnostik umfasst neben Anamnese und körperlicher Untersuchung (inkl. rektaler Austastung) eine kleine Laborroutine, die abdominelle Ultraschalluntersuchung sowie bei Frauen eine gynäkologische Untersuchung. Bei V. a. funktionelle Dyspepsie sollte zudem eine Gastroskopie oder zumindest ein nicht-invasiver Test auf Helicobacter-pylori-Infektion erfolgen. Dominiert eine Diarrhö das Stuhlgangsverhalten, erfordert dies stets eine weitere umfangreiche Diagnostik, um spezifisch therapierbare Ursachen nicht zu übersehen. Hierzu zählen u. a. Atemteste auf Nahrungsmittelintoleranzen, Zöliakie- und Stuhldiagnostik (Mikrobiologie, Entzündungsmarker) sowie Ileokoloskopie inklusive PE-Entnahme. Spricht die Beschwerdesymptomatik nicht auf die nach obiger Diagnostik eingeleitete Therapie an oder sind anamnestisch ausgeprägte Motilitätsstörungen zu vermuten (z. B. Gastroparese, CIPO, „Slow-transit“-Obstipation) sollte eine erweiterte Spezialdiagnostik eingeleitet werden.
Abstract
Diagnosis of functional abdominal complaints in the upper (functional dyspepsia) and lower (irritable bowel syndrome) gastrointestinal (GI) tract, requires the differential diagnostic exclusion of organic diseases. Particularly in the case of existing alarm symptoms, an initial test therapy should not be attempted in cases of dyspeptic symptoms and/or defecation problems.
Each diagnostic evaluation starts with the medical history and physical examinations including rectal digitation, laboratory testing, abdominal ultrasound and gynecological investigations for women. Upper GI endoscopy or at least noninvasive testing for Helicobacter pylori should be performed in patients with dyspeptic symptoms.
If defecation is dominated by diarrhea patients have to undergo thorough testing in order not to overlook specific treatable causes. This includes breath testing for food intolerance, transglutaminase antibodies for coeliac disease, fecal testing (microbiology and inflammation markers) and colonoscopy with biopsies from ileum and colon.
Further investigations are recommended for patients with refractory symptoms or suspected severe motility disturbances, such as gastroparesis, chronic intestinal pseudo-obstruction (CIPO) or slow transit constipation.
