Zusammenfassung

Bei Patienten mit erhöhten Leberwerten, die anamnestisch katecholaminpflichtig waren und invasiv beatmet wurden, sollte auch an eine sekundär sklerosierende Cholangitis „bei kritisch Kranken“ gedacht werden. Im hier besprochenen Fall gelang es, die üblicherweise rasche Progression dieser SSC-Sonderform durch biliäre Sphinkterotomien mit Castextraktion und Antibiotikabehandlung deutlich zu verlangsamen.

Abstract

In patients with increased liver function test values, who had been prescribed catecholamines and invasively ventilated, a secondary sclerosing cholangitis should always be suspected in the critically ill. In the case discussed here the normally rapid progression of this special form of secondary sclerosing cholangitis could be clearly retarded by biliary sphincterotomy with cast extraction and antibiotic treatment.
