Zusammenfassung
Die chronischen Leukämien, insbesondere die Chronische lymphatische Leukämie (CLL), zählen zu den häufigsten malignen hämatologischen Systemerkrankungen. Sowohl die CLL als auch die Chronische myeloische Leukämie (CML) tritt vor allem mit zunehmendem Lebensalter auf. Die klinische Symptomatik ist anfangs unspezifisch, so dass CML und CLL heute häufig per Zufall festgestellt werden. Grundlegende Erkenntnisse in der molekularen Pathophysiologie beider Erkrankungen haben sowohl die Diagnostik als auch die Therapie entscheidend beeinflusst. Zudem erlaubt der Einsatz molekularer Techniken die einfache Diagnosestellung, Prognoseabschätzung und Verlaufsbeurteilung unter Therapie. Vor allem durch den Einsatz zielgerichteter Medikamente haben sich die therapeutischen Möglichkeiten in den letzten Jahren erheblich erweitert und den Stellenwert der allogenen Stammzelltransplantation (allo-SCT) zurückgedrängt. Dennoch stellt die allo-SCT für einen Teil der Patienten weiterhin eine wichtige Therapieoption dar. Diese Übersicht konzentriert sich auf den aktuellen Stand der Diagnostik und Therapie der CML und CLL in der klinischen Praxis.
