Zusammenfassung
Die demographische Entwicklung führt auch zum Anstieg der in der Notfallrettung zu versorgenden älteren Patienten. In diesem ansteigenden Patientenkollektiv ist die Sepsis ein wichtiges Krankheitsbild mit einem schlechten Outcome, und daher müssen sowohl die Strategien in der präklinischen Notfallversorgung, wie auch Diagnostik und die Ersttherapie entsprechend adaptiert werden. Akute Infektionen sind ein häufiger Grund für die Rettungsdienstalarmierung und auch verantwortlich für viele Todesfälle bei alten Menschen. In diesem Zusammenhang kommt der Sepsis als Krankheitsbild in der alternden Gesellschaft eine besondere Bedeutung zu. Der geriatrische Patient ist definiert durch die Multimorbidität, den Funktionalitätsverlust und den zunehmenden Verlust an Selbständigkeit. Die Sepsis ist eine systemische inflammatorische Antwort des Organismus auf eine bakterielle Infektion und nach wie vor mit einer hohen Sterblichkeit verbunden. Daher ist es unerlässlich, die Sepsis sehr früh zu identifizieren. Diagnostische Strategien wie besondere Untersuchungsmethoden, adaptierte Screening- und Assessmentinstrumente müssen entwickelt und eingeführt werden. Ausbildungsprogramme für den Notfallrettungsdienst sind genauso essenziell wie das Erstellen von standardisierten Arbeitsanweisungen für das Krankheitsbild Sepsis. Das Wissen um die speziellen Eigenschaften und Symptome schwerer Infektionen bei alten Patienten, weiterführende Kompetenzen und geriatrische Grundkenntnisse sind notwendig, um die Patienten adäquat versorgen zu können.
Abstract
The demographic shift leads to an increasing number of elderly being treated by emergency services (ES). In this growing collective of elderly patients, sepsis is an important determinant in adverse outcome and thus adapted strategies in preclinical services to adjust diagnostics and treatment are required. Because infections are a common reason to call ES and are also responsible for many deaths among the elderly, sepsis is becoming a major health care problem in the aging society. The geriatric patient is defined by multimorbidity, functional decline and loss of autonomy. Sepsis as a systematic inflammatory response to bacterial infection is still associated with high mortality. Therefore, early detection and initiating of therapeutic bundles is crucial to improve outcomes. Diagnostic strategies and adapted screening and assessment tools must be implemented. Educational programs for physicians and paramedics are as essential as standardized operating procedures for sepsis. Knowledge of the specific characteristics and symptoms of serious infections in the elderly and further research are necessary to be able to ensure the best care for this fragile population.
