Stufenmodell der Schmerzrehabilitation in den BG Kliniken

Zusammenfassung

Die Schmerzrehabilitation der BG Kliniken beinhaltet eine umfassende Behandlung von Patienten mit komplexen Schmerzzuständen. Sie erfolgt in 3 Stufen, abhängig vom Rehabilitationsziel: durch eine erweiterte multimodale Schmerztherapie (eMST), als komplexe stationäre Rehabilitation (KSR) oder seltener als berufsgenossenschaftlich stationäre Weiterbehandlung (BGSW). Die Therapie orientiert sich an dem biopsychosozialen Modell der Schmerzchronifizierung und erfolgt unter Berücksichtigung schmerzverstärkender Kontextfaktoren und des DGUV(Deutsche Gesetzliche Unfallversicherung)-Heilverfahrens. Therapieziele sind Verbesserung von Aktivität, Lebensqualität und Teilhabe unter Schmerzreduktion oder verbesserte Schmerztoleranz bei Vermeidung von sekundären Einschränkungen. Die Behandlung erfolgt mit einem höheren Anteil an individualisierter Therapie sowie höherem personellem Zeitaufwand im Vergleich zu Standardverfahren der DGUV außerhalb von BG Kliniken. Rahmenbedingungen sind daher ein hohes Maß an Interdisziplinarität und Kommunikation. Inhalte sind u. a. Patientenedukation, medikamentöse und nichtsystemische Therapie, Behandlung von therapieassoziierten Komplikationen, Entzugsbehandlung, spezielle invasive und neuromodulative Therapieformen, trainingstherapeutische Maßnahmen, Arbeitsplatz- und Alltags-(ADL[„activities of daily living]“)-Training. Die Verordnung einer eMST erfolgt nach Vorstellung in der Schmerzambulanz, nach „Schmerzabklärung“ oder nach Rücksprache mit dem behandelnden D‑Arzt und bedarf der Genehmigung des zuständigen Kostenträgers. Ein Formtext zur Verordnung ist geplant. Die Verordnung der Schmerzrehabilitation im Rahmen einer komplexen stationären Rehabilitation (KSR) bzw. BGSW erfolgt mittels der bekannten Formtexte mit entsprechenden Vermerken.

Abstract

Pain rehabilitation in BG Kliniken (Hospital Group of the Statutory Accident Insurance) comprises an extensive treatment program for patients with complex pain syndromes. The treatment incorporates three stages depending on the aim of rehabilitation by advanced multimodal pain treatment (eMST), as complex inpatient rehabilitation (KSR) and to a lesser extent as a BG inpatient extended treatment program (BGSW). Treatment is based on the biopsychosocial model of chronic pain and is carried out taking environmental factors that increase pain and the German statutory accident insurance (DGUV) procedure into consideration. The main goals of treatment are improvement in activity, quality of life and participation. This is achieved by reduction of pain or improved pain tolerance by avoiding secondary limitations. In particular, the treatment provides a higher proportion of individualized treatment compared to the standard DGUV procedure as conducted in non-BG hospitals. Hence, a multidisciplinary pain management in a well-orchestrated manner is indispensable. This contains patient education, pharmacological and non-systemic treatment, treatment of disease-related complications, treatment of withdrawal, special invasive and neuromodulative treatment, exercise therapy, training of activities of daily living and work-related training. The prescription of eMST is generally feasible after patient presentation to a pain outpatient clinic and pain assessment or in agreement with the accident insurance consultant as approval for coverage of costs has to be obtained from the insurance responsible prior to initiating treatment. A standardized form for prescription is in progress. For prescription of pain rehabilitation in KSR and BGSW treatment programs, a corresponding form is ubiquitously available.
