Zusammenfassung
Vaskuläre Malformationen sind Läsionen mit unterschiedlichen Charakteristika. Die korrekte Diagnose kann mitunter schwierig sein. Wichtig ist hierbei auch, dass die korrekte Nomenklatur verwendet wird. Die Magnetresonanztomographie (MRT) ist als bildgebende Modalität am ehesten geeignet, um eine vaskuläre Malformation korrekt einteilen zu können. Mithilfe der MRT lässt sich sowohl die Ausdehnung einer Angiodysplasie als auch deren Lagebeziehung zu anderen wichtigen anatomischen Strukturen bestimmen. Ferner ist mithilfe der MRT eine funktionelle Analyse der beteiligten vaskulären Strukturen möglich. Insbesondere die dynamische MR-Angiographie mit gadoliniumhaltigem Kontrastmittel liefert wertvolle hämodynamische Informationen und ermöglicht es, zwischen Malformationen mit niedrigem Durchfluss („low flow“) und solchen mit hohem Durchfluss („high flow“) zu unterscheiden. Zu der Gruppe der Gefäßmalformationen mit niedrigem Durchfluss gehören venöse, lymphatische, kapilläre oder gemischte Läsionen. Bei den Gefäßmalformationen mit hohem Durchfluss wird die arteriovenöse Malformation (AVM) von der arteriovenösen Fistel (AVF) unterschieden. Die MRT bildet eine solide Grundlage bei der Therapieplanung vaskulärer Malformationen.
Abstract
Vascular malformations are lesions with variable characteristics. Finding the correct diagnosis and therapy can be challenging. In this context it is fundamental to use the correct nomenclature. Magnetic resonance imaging (MRI) is the most suitable modality to correctly classify a vascular malformation as MRI accurately demonstrates both the extent of the lesion and its anatomical relationship to adjacent structures and tissues. Furthermore, MRI facilitates functional and hemodynamic vessel analysis including dynamic time-resolved gadolinium contrast material-enhanced three dimensional MR angiography. This is essential to differentiate vascular malformations with low flow, e.g. venous, lymphatic, capillary, or mixed lesions, from lesions with high flow, e.g. arteriovenous malformations (AVM) or arteriovenous fistulas (AVF). The MRI technique forms a solid foundation for both diagnostic imaging and therapy planning of vascular malformations.
