Zusammenfassung
Bei Verschlüssen der femorodistalen Strombahn kann sowohl endovaskulär als auch offen-chirurgisch vorgegangen werden. Die aktuellen TASC-Empfehlungen sehen die endovaskuläre Behandlung bei kurzstreckigen Verschlüssen bis zu 5 cm unter Einsatz eines Stents als erfolgversprechend an. Offen-chirurgisch ist regelhaft aufgrund der Lokalisation ein Interponat oder Bypass notwendig. Die Ergebnisse sowohl der endovaskulären als auch der offen-chirurgischen Therapie sind mit 70–80% innerhalb des ersten Jahres moderat. Seit etwa 22 Monaten (August 2007) ist, ähnlich der Heparin beschichteten konventionellen PTFE-Prothese, auch eine Viabahn-Endoprothese mit propatenbioaktiver Oberfläche klinisch einsetzbar. Diese Propatenbeschichtung führt im konventionellen Bereich im ersten Jahr zu einer signifikanten Verbesserung der Offenheitsraten auf bis zu 97%. Wir zeigen anhand des beschriebenen Falls, dass diese Prothese erfolgreich implantiert werden kann. Die Langzeitergebnisse bleiben abzuwarten. Aus diesem Grund sollte diese Prothese auch nur dann zum Einsatz kommen, wenn ihre exakte klinische Nachsorge sichergestellt ist.
Abstract
Femorodistal occlusion can be treated with bypasses or even with endovascular methods nowadays. The TransAtlantic Inter-Society Consensus (TASC) paper recommends the endovascular option for short occlusions up to 5 cm. The primary patency rates for both options are moderate: 70–80%. Since August 2007, the Viabahn Endoprosthesis with Propaten bioactive surface has been in clinical use. In open surgery, the surface activation by means of Propaten can improve the patency rates of femorodistal bypasses up to 97% during the first year after implantation. We show that implantation of the Viabahn Endoprosthesis is feasible; however, because midterm and long-term results are lacking, the patients should be followed up closely.
