Zusammenfassung
Hintergrund
Die Empfehlungen zur Strahlentherapie bei Knochenmetastasen der S3-Leitlinie Supportivtherapie für onkologische Patienten werden dargestellt und vor dem Hintergrund der S3-Leitlinie Palliativmedizin für nicht heilbare Krebspatienten diskutiert.
Ergebnisse
Zur Schmerzlinderung bei Knochenmetastasen ohne Rückenmarkkompression oder Fraktur ist die Einzeitbestrahlung ebenso wirksam wie fraktionierte Konzepte. Bei Patienten mit voraussichtlich sehr limitierter Lebenszeit (wenige Wochen bis Monate) ist eine Einzeitbestrahlung eine sinnvolle Option. Bei Patienten mit längerer Überlebensprognose (mindestens 3–6 Monate) ist die Wiedergewinnung der Stabilität/der Stabilitätserhalt ein realistisches Ziel. In dieser Situation sind fraktionierte Konzepte der Einzeitbestrahlung überlegen. Bei Patienten in gutem Allgemeinzustand und ausreichender Prognose soll bei neu aufgetretener, funktionell relevanter Rückenmarkkompression eine notfallmäßige Dekompression, gefolgt von einer Radiatio durchgeführt werden. Andernfalls ist eine primäre Strahlentherapie indiziert. Prognosescores können die ärztliche Prognoseeinschätzung unterstützen. Die Therapieentscheidungen sollten interdisziplinär und gemeinsam mit dem Patienten bzw. seinem Vertreter getroffen werden.
Schlussfolgerung
Es ist zu hoffen, dass die hier im deutschen Kontext erstmals auf S3-Leitlinien-Niveau konsentierten Empfehlungen zeitnah Eingang in die Alltagspraxis finden.
Abstract
Background
The recommendations for radiation therapy of bone metastases from the S3 guidelines on supportive therapy for cancer patients are presented and discussed in the light of the S3 guidelines on palliative care for incurable cancer patients.
Results
The results are: for pain relief in bone metastases without spinal cord compression or fracture, one-time radiation is as effective as fractionated concepts. For patients with very limited prognosis (few weeks to months), one-time radiation is a valid option. For patients with a better survival prognosis (at least 3–6 months), recovering or maintaining stability is a realistic goal. In these situations, fractionated concepts are superior to one-time radiation. Patients in a good general condition and adequate prognosis should be treated by immediate surgical decompression followed by radiotherapy in the case of newly occurring, functionally relevant spinal cord compression. Otherwise, primary radiotherapy is indicated. Prognosis scores can support the physician’s prognosis assessment. Therapy decisions should be interdisciplinary and also made together with the patient or a representative.
Conclusion
We hope that the recommendations implemented at the German S3 guidelines level for the first time, will soon find their way into everyday practice.
