Abstract
BACKGROUND: According to study findings, pain in children and adolescents in hospital is often underestimated or not identified. No epidemiological data on the prevalence or cause of pain in a general paediatric population are available for Austria. AIM: This study aims at presenting the frequency, intensity and cause of pain in children and adolescents aged between 5 and 17 years in an Austrian university hospital. METHOD: In 2009 data were collected through semistandardised interviews at seven paediatric departments. In 200 children and adolescents, the prevalance and intensity of pain at the time of the interview as well as in the last 24 hours were registered. Furthermore, the cause of pain was analysed. RESULTS: The prevalance of pain was 79.5% in the last 24 hours before the interview. Almost half of the children and adolescents (45.5%) reported to suffer from pain at the time of the interview. The average intensity of pain during the last 24 hours was described as "moderate" or "severe" by 43%. More than half of the participants (61.5%) had "moderate" or "severe" pain as strongest pain intensity in the last 24 hours. The most frequent causes of pain were the disease (e.g. pneumonia) or a surgical treatment (e.g. appendectomy). CONCLUSION: The frequent occurrence of clinical significant levels of pain indicates deficits in pain management. An effective pain management can spare children and adolescents pain, especially severe levels of pain. This requires the recognition, systematic assessment and an appropriate treatment of pain.
Abstract
PROBLEMSTELLUNG: Schmerzen bei Kindern und Jugendlichen im Krankenhaus werden laut Ergebnissen von Studien oft unterschätzt bzw. nicht erkannt. Für Österreich liegen keine epidemiologischen Daten zur Schmerzprävalenz und Ursache von Schmerzen in einer pädiatrischen Gesamtpopulation vor. ZIEL: Mit dieser Untersuchung sollten die Häufigkeit, Stärke und Ursachen von Schmerzen bei Kindern und Jugendlichen zwischen 5 und 17 Jahren an einem österreichischen Universtätsklinikum dargestellt werden. METHODE: Die Datenerhebung erfolgte 2009 mittels halbstandardisierter Interviews auf sieben pädiatrischen Stationen. Bei 200 Kindern und Jugendlichen wurden Schmerzprävalenz und Schmerzintensität zum Zeitpunkt des Interviews sowie in den letzten 24 Stunden erfasst. Weiters wurde die Ursache der Schmerzen erhoben. ERGEBNISSE: Die Schmerzprävalenz in den letzten 24 Stunden vor dem Interview lag bei 79,5%. Fast die Hälfte der Kinder und Jugendlichen (45,5%) gab Schmerzen zum Zeitpunkt des Interviews an. Die durchschnittliche Schmerzintensität in den letzten 24 Stunden beschrieben 43% mit "mittelstark" oder "stark". Mehr als die Hälfte der Probanden hatte als stärkste Schmerzintensität in den letzen 24 Stunden "mittelstarke" oder "starke" Schmerzen. Häufigste Schmerzursachen waren die Erkrankung (z.B. Pneumonie) oder eine operative Behandlung (z.B. Appendektomie). SCHLUSSFOLGERUNG: Das häufige Vorkommen von therapiepflichtigen Schmerzen weist auf Defizite im Schmerzmanagement hin. Durch ein effektives Schmerzmanagement können Kindern und Jugendlichen Schmerzen, insbesondere starke Schmerzen, erspart werden. Dies erfordert, dass Schmerzen erkannt, systematisch eingeschätzt und eine entsprechende Therapie durchgeführt wird.
