Zusammenfassung
Chronischer Rückenschmerz („chronic low back pain“, cLBP) ist ein unverändert häufiges Krankheitsbild. Die Tendenz ist trotz ständiger Weiterentwicklung diagnostischer und therapeutischer Verfahren steigend. Eine funktionelle Instabilität der Lendenwirbelsäule (LWS) stellt einen Teil der Patienten mit cLBP dar. Segmentale Instabilität spielt in der täglichen Praxis bei der Rehabilitation von Patientinnen und Patienten mit chronischem Rückenschmerz eine große Rolle. Die Insuffizienz der tiefenstabilisierenden Muskulatur ist hierbei ein regelhafter Befund. Die klinische Diagnostik ist vielfältig, wobei eine große Zahl an Tests zur Verfügung steht. Für den klinischen Alltag ist jedoch ein einfacher und reliabler klinischer Algorithmus in der Diagnosefindung Voraussetzung für ein effektives und befundorientiertes Arbeiten. Der Artikel befasst sich mit Diagnostik- und Behandlungskonzepten bei funktioneller Instabilität der LWS, welche die Autoren an ihrer Klinik etabliert haben. Diese Konzepte sind eingebettet in therapeutische Standards, die einen multifokalen interdisziplinären Ansatz verfolgen.
Abstract
Chronic lower back pain (cLBP) continues to be a common clinical picture. Despite continuous advancements in diagnosis and treatment, the trend is still increasing. Functional instability of the lumbar spine is observed in a proportion of patients with cLBP. Segmental instability plays an important role in the daily practice of rehabilitating patients with chronic back pain. Insufficiency of the deep stabilizing system musculature is a common diagnosis. Clinical diagnostics are diverse, with many different tests available. However, for clinical routine, an easy and reliable diagnostic clinical algorithm is required to guarantee an effective and diagnosis-oriented workflow. This article addresses diagnostic and therapeutic concepts for functional instability of the lumbar spine that the authors have established at their clinic. These concepts are embedded in therapeutic standards pursuing a multifocal interdisciplinary approach.
