Zusammenfassung
Hintergrund
Die Behandlung von Mammakarzinompatientinnen hat sich v. a. durch Neuentwicklungen im Bereich der Systemtherapie enorm verbessert, sodass ein mehrjähriges Überleben für viele Patientinnen erreichbar ist. Neuere Erkenntnisse im Bereich der Tumorbiologie und Beobachtungen klinischer Verläufe haben zur Definition des Stadiums der Oligometastasierung geführt.
Fragestellung
Welche Rolle spielt die stereotaktische Radiotherapie in der Behandlung von Patienten mit oligometastasiertem Mammakarzinom?
Material und Methode
Durchgeführt wurde eine halbstrukturiere Literaturrecherche.
Ergebnisse
Die stereotaktische Radiotherapie ist ein lokal äußerst effektives und sicheres Verfahren zur Behandlung von Hirn‑, Lungen‑, Leber- und Knochenmetastasen. Eine erste entitätenübergreifende randomisierte Phase-II-Studie (SABR-COMET) hat eine Verbesserung des Gesamtüberlebens in der oligometastasierten Situation durch eine stereotaktische Radiotherapie zeigen können. Präklinische und erste klinische Daten deuten auf eine immunstimulierende Wirkung der stereotaktischen Radiotherapie hin.
Diskussion
Entitätenspezifische randomisierte Studien sind dringend erforderlich. Die OLIGOMA-Studie wird den Stellenwert der stereotaktischen Radiotherapie als Ergänzung zur medikamentösen Standardtherapie bei Patientinnen mit bis zu 5 Metastasen eines Mammakarzinoms untersuchen.
Abstract
Background
New developments in the systemic treatment of patients with metastatic breast cancer have led to enormous improvements, rendering a survival time of several years achievable for many patients. New insights in tumor biology and observation of clinical cases have led to the definition of oligometastases as a distinct disease stage.
Objectives
To analyse the potential role of stereotactic radiotherapy in the treatment of patients with oligometastatic breast cancer.
Methods
Semi-structured literature review.
Results
Stereotactic radiotherapy is a highly effective and safe local treatment option for patients with brain, lung, liver and bone metastases. A randomized clinical phase II-trial (SABR-COMET) enrolling patients with oligometastases from various primary tumors showed an improvement in overall survival from stereotactic radiotherapy. Preclinical and limited clinical data suggest immune-activating effects of stereotactic radiotherapy.
Discussion
Further clinical trials enrolling patients with specific tumor types are needed. The OLIGOMA-trial will further analyze the role of stereotactic radiotherapy in addition to standard systemic therapy in patients with breast cancer and up to 5 metastatic sites.
