Zusammenfassung
Palliativmedizinische Maßnahmen spielen eine wesentliche Rolle bei der Versorgung von Patientinnen mit fortgeschrittenen gynäkologischen Tumoren sowie Mammakarzinomen. Neben einer empathischen und ehrlichen Kommunikation ist eine adäquate Schmerz- und Symptomkontrolle der Patientinnen essenziell. Randomisierte Studien zeigen, dass eine frühzeitige Einbeziehung der Palliativmedizin in der onkologischen Behandlung von Patientinnen mit fortgeschrittenen Krebserkrankungen nicht nur die Lebensqualität, sondern auch das Überleben verbessern kann. Insgesamt ist Palliativmedizin für die meisten Patientinnen mit fortgeschrittenen gynäkologischen Tumorerkrankungen und Mammakarzinom hilfreich und sollte frühzeitig wie niederschwellig angeboten werden.
Abstract
Palliative care plays an important role in the care of patients with advanced gynecological tumors and breast cancer. In addition to an empathic and honest communication, adequate pain and symptom control of the patients is essential. Randomized studies have shown that early integration of palliative care into the oncological treatment of patients with advanced cancer not only improves the quality of life but can also improve survival. Overall, palliative care is helpful for most patients with advanced gynecological tumors and breast cancer and should be provided early and as a low threshold offer.

Eine 48-jährige Patientin mit hepatisch und ossär metastasiertem Mammakarzinom, Erstdiagnose vor 3 Jahren (pT2, pN1, M0, ER +, PR −, HER-2 FISH +) entwickelt einen „cancer en cuirasse“. Zuletzt erhielt sie Lapatinib + Capecitabine. Die 2‑fache Mutter leidet unter Schmerzen und Atemnot. Sie kann das Haus nicht mehr verlassen, weil sich im Bereich der thorakalen Tumorausbreitung eine breitflächige nässende Wunde entwickelt hat. Was ist das korrekte Vorgehen in diesem Fall?
Wechsel auf eine antiöstrogene Therapiestrategie, Behandlung von Schmerzen und Atemnot, Ansprechen der malignen Wunde beobachten
Eine chirurgische Intervention kann hier zu nachhaltiger Linderung führen, z. B. Exzision oder Elektrochemotherapie sind ausreichend.

Ileus ist eine typische Spätkomplikation des Ovarialkarzinoms und kann auch bei anderen fortgeschrittenen Tumorerkrankungen z. B. mit peritonealer Fernmetastasierung auftreten. Welche Aussage trifft nicht zu?
Zu den typischen Symptomen des Ileus gehören abdominelle Schmerzen. Die analgetische Therapie muss auf Nichtopioide beschränkt bleiben, weil Opioide sie die Darmmotilität hemmen.
Zur Reduktion der Sekretion wird Butylscopolamin und/oder Octreotid (Somatostatinanalogon) eingesetzt.
Um das Erbrechen zu reduzieren, sollte eine großlumige Magensonde gelegt werden, diese kann bei persistierenden Beschwerden durch eine PEG ersetzt werden.
Auch bei Patientinnen mit fortgeschrittenen Tumorleiden gilt, dass ein Ileus rasch operiert werden muss („Über einen Ileus darf die Sonne nicht auf- oder untergehen!“), da die Patientinnen andernfalls in wenigen Tagen versterben.
Faktoren, die gegen eine Operation sprechen sind: vorausgegangene Operation mit Inoperabilität, diffuse Metastasierung, Mitbeteiligung proximaler Magen, rasch wiederauftretender Aszites
Eine 69-jährige Patientin mit einem primär ossär metastasierten Mammakarzinom klagt über progrediente starke Schmerzen in der Wirbelsäule. Welche Therapieoptionen sollten der Patientin zunächst angeboten werden?
