Zusammenfassung
Die nicht-parasitäre Chylurie ist selten. Die Spontanheilung liegt bei 50%. Für die konservative Behandlung stehen die Diät mit mittelkettigen Triglyzeriden, die Lymphangiographie sowie die Instillation von sklerosierenden Silbernitratlösungen zur Verfügung. Bei Versagen der konservativen Maßnahmen stellt die offene oder die retroperitoneoskopische Nephrolyse die Therapie der Wahl dar. Wir stellen einen Fall einer spontan aufgetretenen nicht-parasitären Chylurie der linken Niere vor, welche 2-malig endoskopisch mit dem Neodym-YAG-Laser behandelt wurde. Dies wurde bisher in der Literatur noch nicht beschrieben. Die Chylurie konnte hiermit über mehrere Monate zum Versiegen gebracht werden. Das temporäre Verschwinden der Chylurie nach lokaler Laserbehandlung ermutigt zur weiteren Entwicklung dieser schonenden Therapieform nach Versagen der konservativen Therapieoptionen.
Abstract
Nonparasitic chyluria is rare. Spontaneous remission occurs in 50% of patients. Conservative treatment includes diet with medium-chain triglycerides, lymphangiography, and the instillation of sclerosing solutions of silver nitrate. If conservative management fails, open or retroperitoneoscopic nephrolysis is the treatment of choice. We present a case of spontaneous recumbent nonparasitic chyluria from the left kidney treated twice by endoscopic application of neodymium-YAG laser. This is new to the literature. The chyluria disappeared for several months. The temporary cessation of chyle flow after local laser application should encourage further use of this treatment modality after failure of conservative treatment.
