Zusammenfassung

Endoskopische Interventionen am Gallengang sind definiert durch die Beseitigung eines Abflusshindernisses, wie bei der Choledocholithiasis, oder die Optimierung des Galleflusses durch Drainageverfahren mittels Stents. Ist die ausschließlich diagnostische endoskopisch retrograde Cholangiographie (ERC) mittlerweile obsolet, so stellt aufgrund des minimal-invasiven Ansatzes bei entsprechender Indikation die therapeutische ERC die Methode der Wahl zur Intervention dar. Bei fehlender Möglichkeit eines Zugangs in das Gallengangsystem über die Papille bieten neuerdings für den geübten Endoskopiker endosonographisch gesteuerte Verfahren eine sinnvolle Alternative.

Abstract

Endoscopic interventions in the biliary system are defined by the elimination of an outflow obstruction such as in choledocholithiasis or the optimization of bile flow by means of stent drainage. Exclusive use of diagnostic endoscopic retrograde cholangiography (ERC) is now obsolete, but due to its minimally invasive approach, therapeutic ERC is the method of choice for intervention in appropriate indications. If access to the biliary system via the papilla is not possible, approaches as guided by endoscopic ultrasound appear to be a valid option for the experienced endoscopist.
