Zusammenfassung
Hintergrund
In der Diagnostik und Therapie von tachykarden Herzrhythmusstörungen konnten in den letzten 20 Jahren bei Kindern und Jugendlichen deutliche Fortschritte erzielt werden. Die elektrophysiologische Untersuchung und Katheterablation von supraventrikulären Tachykardien erwiesen sich als äußerst effektive und sichere Verfahren, sodass sie heute bei entsprechender Symptomatik bei größeren Kindern und Jugendlichen den Familien als primäre Therapie angeboten werden können.
Ziel des Beitrags
Es werden zunächst kurz die klinische Symptomatik sowie der natürliche Verlauf paroxysmaler supraventrikulärer Tachykardien im Kindesalter erläutert. Anschließend werden die Prinzipien der elektrophysiologischen Untersuchung und der Katheterablation inklusive der bislang erzielten Ergebnisse bei Kindern komprimiert dargestellt. Darauf basierend werden typische klinische Fragestellungen anhand der aktuellen Leitlinie der Deutschen Gesellschaft für Pädiatrische Kardiologie diskutiert und die aktuellen Empfehlungen zur Vorgehensweise bei asymptomatischen Kindern mit einem Präexzitationssyndrom dargestellt.
Abstract
Background
In the last 20 years, significant progress could be achieved in the diagnosis and therapy of tachyarrhythmias in children and adolescents. Electrophysiological study and catheter ablation of supraventriuclar tachycardia have proved to be extremely effective and safe procedures. Therefore, this therapy can be offered today as first-line therapy to families with symptomatic older children and adolescents.
Aim of article
This report briefly illustrates symptoms and natural course of paroxysmal supraventricular tachycardia in the young. Subsequently, principles of electrophysiological study and catheter ablation including the results as achieved so far in children are presented. Typical clinical situations are discussed according to the current guidelines of the German Society for Pediatric Cardiology. Finally, for pediatric patients with asymptomatic ventricular preexcitation current guidelines are discussed.
