Pacemaker implantation in pallid breath-holding spells

Zusammenfassung

Affektkrämpfe sind eine häufige Ursache für plötzliche Bewusstseinsverluste im Säuglings- und Kleinkindesalter. Wir berichten über einen 10 Monate alten Jungen, der wiederholte lebensbedrohlich erscheinende Ereignisse mit Bewusstlosigkeit, Augenverdrehen, versteiften Extremitäten, perioraler Zyanose und überwiegender Blässe am übrigen Körper darbot. Während der Langzeit-EKG-Aufzeichnung (EKG: Elektrokardiogramm) kam es zu 2 affektgebundenen Ereignissen, bei denen jeweils ein Sinusarrest von 10–11 s Dauer nachgewiesen werden konnte. Deshalb wurde im Alter von 1 Jahr ein Herzschrittmacher implantiert, was die vollständige Behebung der klinischen Symptomatik zur Folge hatte.

Abstract

Breath-holding spells are a frequent cause of syncope in infancy. We report on a 10-month-old boy presenting with recurrent episodes of apparent life-threatening events associated with loss of consciousness, rolling eyes, stiffened extremities, perioral cyanosis and predominant pallor of the remaining body. During Holter (long-term) ECG examination two affect-induced events with a sinus arrest of 10–11 s occurred. Therefore at the age of 1 year a pacemaker was implanted with a favorable effect on the clinical progress.
