Zusammenfassung
Fallbeschreibungen von erworbenen traumatischen Laryngomalazien sind in der Literatur äußerst selten. Wir berichten über eine 47-jährige Patientin, die infolge eines transzervikalen Zugangswegs bei einer Operation an der Halswirbelsäule in Intubationsnarkose über inspiratorische Atemnot klagte. Die Laryngostroboskopie zeigte eine weiche und bei Inspiration rückverlagerte Epiglottis. Zusätzlich wurde erstmalig bei diesem Krankheitsbild eine dynamische MRT durchgeführt, welche eine traumabedingte erhöhte Mobilität des oberen Epiglottisrands darstellen konnte. Nach einer partiellen laserchirurgischen Epiglottektomie sistierten die Beschwerden der Patientin.
Abstract
Reports on acquired traumatic laryngomalacia appear only rarely in the literature. We report a case of a 47-year-old patient with breathlessness after a transcervical approach to the spine during general anesthesia with intubation. Laryngostroboscopy revealed a smooth and, during inspiration, retroflexed epiglottis at the posterior pharyngeal wall. Additionally, dynamic magnetic resonance imaging for the first time showed hypermobility of the upper edge of the epiglottis. After partial laser epiglottectomy, the patient’s complaints disappeared.
