Zusammenfassung
Laparoskopische Operationen über nur einen Trokar (Single-port-Technik) verfolgen wie NOTES „natural orifice transluminal endoscopic surgery“ das Ziel, das Zugangstrauma durch weitere Minimierung der Hautinzisionen zu verringern. Zur Durchführung sind spezielle Trokare und Instrumente notwendig, die in dieser Arbeit in ihrer Anwendung und Funktion dargestellt werden. Das Indikationsspektrum entspricht dem der „konventionellen“ laparoskopischen Chirurgie. Die bisherigen Publikationen zeigen, dass Operationen in Single-port-Technik sicher und effizient durchführbar sind, wobei ein höherer operativer Schwierigkeitsgrad und höhere Kosten berücksichtigt werden müssen. Für den Nachweis signifikanter Vorteile fehlen bisher randomisierte Studien.
Abstract
Laparoscopic operations using only one trocar (single-port technique) have the aim to further minimize the trauma of access incisions similar to NOTES. To carry this out special trocars and instruments are needed which are presented in this article with respect to application and function. The range of indications corresponds to the conventional laparoscopic surgery. Recent publications have shown that operations using a single-port technique are safe and efficient. Greater technical difficulties and higher costs must be considered. For the identification of significant advantages, randomized studies are still lacking.
