Zusammenfassung
Operationsziel
Vollständige Gelenkflächendarstellung bei multifragmentärer Gelenkflächendestruktion im Rahmen von uni- oder bikondylären Tibiakopffrakturen als Voraussetzung für eine anatomische Rekonstruktion, um das posttraumatische Arthroserisiko zu reduzieren.
Indikationen
Unikondyläre laterale oder mediale sowie bikondyläre intraartikuläre Tibiakopffrakturen mit zentralen und/oder dorsalen Frakturanteilen; multifragmentäre Destruktion im medialen oder lateralen Tibiaplateau mit Dislokation oder Stufenbildung >2 mm.
Kontraindikationen
Infektionen im Zugangsbereich, femorale Kondylenfraktur, intraligamentäre Rupturen des Innen- und Außenbandes oder der posterolateralen Ecke.
Operationstechnik
Medial: Zugang über den medialen oder anteromedialen Zugang; lateral: über den antero- oder posterolateralen Zugang zur osteosynthetischen Versorgung der Tibiakopffrakturen. Scharfe Präparation bis auf die medialen/lateralen ligamentären Begleitstrukturen mit anschließender Darstellung des medialen/lateralen Femurepikondylus. Medial: ca. 2 × 2 × 1 cm große Osteotomie des medialen Femurepikodylus als femoralen Innenbandansatz. Lateral: Osteotomie eines etwa 1 × 1 × 0,5 cm großen Knochenblocks des lateralen Femurepikodylus als femoralen Außenbandansatz, entweder unter Schonung oder inkl. der ventral im Sulcus popliteus verlaufende Popliteussehne. Dabei sollte eine Verletzung der artikulären Kondylenbereiche vermieden werden.
Weiterbehandlung
Frühfunktionelle Nachbehandlung ohne Ruhigstellung in einer gelenkübergreifenden Rahmenorthese mit 10- bis 20-kg-Teilbelastung an Unterarmgehstützen frakturabhängig für 6–12 Wochen.
Ergebnisse
Sehr gute Darstellbarkeit multifragmentärer Gelenkflächendestruktionen mit postoperativ anatomischer Rekonstruktion komplexer Frakturmuster ohne postoperative Begleitinstabilitäten.
Abstract
Objective
Complete visualization of the articular surface in comminuted uni- or bicondylar tibial plateau fractures as a prerequisite for anatomical reconstruction to reduce the risk of posttraumatic osteoarthritis.
Indications
Unicondylar lateral or medial as well as bicondylar intra-articular tibial plateau fractures with central and/or dorsal fracture lines; comminuted destruction of the medial or lateral tibial plateau with dislocation of >2 mm.
Contraindications
Critical soft tissue in the approach area, femoral condylar fracture, intraligamentous ruptures of the medial or lateral ligaments or the posterolateral corner.
Surgical technique
Medial: via the medial or anteromedial approach; lateral: via the antero- or posterolateral approach for open reduction and internal fixation of the tibial plateau fracture. Sharp dissection down to the medial/lateral ligamentous accompanying structures with subsequent presentation of the medial/lateral femoral epicondyle. Medial: approximately 2 × 2 cm osteotomy of the medial femoral epicondyle. Lateral: osteotomy of an approximately 1 × 1 × 0.5 cm bone block of the lateral femoral epicondyle either with protection or including the popliteus tendon running ventrally in the sulcus popliteus. In this case, a violation of the articular condyle should be avoided.
Postoperative management
Early functional posttreatment with full mobilization and 10–20 kg partial load bearing on forearm crutches, fracture-dependent for 6–12 weeks.
Results
Very good visualization of the comminuted articular surface with postoperatively anatomical reconstruction of complex fracture patterns without postoperative concomitant instabilities.

Ein 30-jähriger Motorradfahrer ohne bekannte Knochenerkrankung erlitt eine traumatisch bedingte, mehrfragmentäre, unikondyläre Tibiakopffraktur, welche die posterolaterolateralen, posterolaterozentralen und anterolaterolateralen Segmente betraf. Nach computertomographisch begründeter Strategiefestlegung erfolgte die operative Versorgung über den posterolateralen Zugang nach Frosch, welcher um die laterale femorale Epikondylusosteotomie erweitert wurde. Die postoperative Computertomographie bestätigte die anatomische Gelenkflächenrekonstruktion und die stabile Fixation. Wie sollte die postoperative Versorgung bei diesem Patienten am sinnvollsten aussehen?

Im Rahmen der operativen Darstellung der lateralen Ligamentstrukturen sollte der N. peronaeus immer dargestellt werden.
Neben der Osteotomie des lateralen Femurepikondylus ist der zusätzliche Einschluss des femoralen Popliteussehnenansatzes möglich.
Der erweiterte laterale Zugang ermöglicht lediglich eine Verbesserung der Visualisierung anterolaterozentraler Gelenkflächenanteile.
Neben der lateralen Epikondylusosteotomie sollte zusätzlich auch immer der Außenmeniskus an seinem Hinter- oder Vorderhorn gelöst werden.
Der erweiterte laterale Zugang sollte nur im Ausnahmefall Anwendung finden, da das Risiko einer postoperativen posterolateralen Rotationsinstabilität hoch ist.
