Emergency management of perforated sigmoid diverticulitis and hemorrhage

Zusammenfassung

Die perforierte Sigmadivertikulitis und die Sigmadivertikelblutung erfordern ein effektives und interdisziplinäres Notfallmanagement. In den letzten Jahren wurden neue Therapieansätze und Behandlungsstrategien, wie die laparoskopische Resektion oder Lavage, die primäre Kontinuitätswiederherstellung, das nichtoperative Management oder die „damage control surgery“ in zahlreichen Studien evaluiert. Anhand der wissenschaftlichen Evidenz können differenzierte Behandlungsalgorithmen abgeleitet werden, um das individuelle Management zu verbessern. Hierin besteht das Potenzial, die Morbidität und Mortalität der Erkrankungen in Zukunft weiter verringern zu können.

Abstract

A perforated sigmoid diverticulitis and sigmoid diverticular hemorrhage require an effective and interdisciplinary emergency management. New treatment concepts, such as laparoscopic resection and lavage, primary reconstruction of continuity by anastomosis, non-operative management or damage control surgery have been recently evaluated in various clinical studies. New scientific evidence has enabled the development of sophisticated treatment algorithms to improve individual management. This has the potential to be able to further reduce the morbidity and mortality of these diseases in the future.
